Registration Requirements

2013 -2014
1. Withdrawal Form - Only if withdrawing during the school year.
2. Last Report Card
3. Birth certificate
4. Social Security card
5. Immunization record needed at time of enrollment (Will need to be on Georgia form #3231 within 30 days. Ear, Eye and Dental form #3300 is due within 120 days).
6. Proof of Residency - Current Georgia Power bill, mortgage deed or signed lease agreement, and current driver’s license with address listed.
7. Custody/Guardianship paper or court orders (if applicable)

6th Grade Orientation 8/13/13 - 6:00 p.m. – 7:30 p.m.
7th & 8th Grade Orientation 8/14/13 – 6:00 p.m. – 7:30 p.m.
